Step 1 - Please download a referral form.
Step 2 - Indicate "Ravindran Research" and the study of interest under Reason for Referral (in addition to relevant information).
Step 3 - A printout of your patient's pharmacy records must accompany the referral
Step 4 - Email completed form to ravindranlab@camh.ca
Step 5 - Your referral may require additional clarification or may be met with the response "approved”.
Step 6 - Once approved, please fax completed form to 416-979-6815
